    • Patients recovering from heart or lung conditions gain control over their health with nationally certified NorthBay Cardiac & Pulmonary Rehabilitation programs. Comprehensive, nurse-supervised exercise and education programs help patients recover quickly and safely, gaining self-confidence and strength to return to daily activities. Patients with new and chronic illnesses learn to manage their disease and address risk factors to enhance quality of life and prevent disability.

    Who can benefit from Cardiac and Pulmonary Rehabilitation?

    Patients with the following conditions may benefit from referral to the program:

    • Heart attack
    • Stent deployment
    • Heart surgery
    • Angina (chest pain)
    • Congestive heart failure
    • Beginning stages or high risk of heart disease
    • Peripheral arterial disease
    • COPD (Chronic Obstructive Pulmonary Disease)
    • Asthma
    • Chronic Bronchitis
    • Pulmonary Fibrosis
    • Heart and/or Lung Transplant
    • Diabetes

     Benefits include:

    • Improved muscle strength, endurance and flexibility
    • Lower blood pressure and heart rate
    • Support from others with heart and lung disease
    • Reduction in total cholesterol and increase in HDL "good" cholesterol
    • Reduction in weight and body fat
    • Independence and confidence through education and self-direction
    • Earlier return to leisure activities, work and activities of daily living


    Cardiac Rehab Program

    Designed for patients recovering from heart disease and cardiac events such as heart attacks, post PTCA/stents, valve surgery or bypass surgery. Patients frequently admitted with CHF or angina can also benefit significantly. Patients admitted to this program can expect:

    • Individual exercise programs supervised by registered nurses specially trained in therapeutic exercise
    • A comprehensive education program
    • Telemetry monitoring
    • Classes 3 days a week for 4 to 12 weeks
    • Body fat analysis pre and post program are shared with each participant.

    Cardiac Rehab is covered by most insurance if ordered by the physician.

    Pulmonary Rehabilitation Program

    Patients with lung disorders such as COPD, Asthma, Pulmonary Fibrosis or Chronic Bronchitis may be referred to this program. Pulmonary Rehabilitation is also required prior to lung transplantation to prepare for surgery. Patients admitted to this program can expect:

    • Individual exercise program supervised by registered nurses, specially trained in therapeutic exercise
    • Patients attend classes taught by a team of nurses, respiratory therapists, a dietitian and social worker
    • Telemetry and oxygen saturation monitoring keeps patients safe while exercising
    • 12 weeks of therapy, with classes 3 times a week
    • Pulmonary rehabilitation is covered by most insurance if ordered by the physician.

    Cardiac Wellness Program 

    This comprehensive education and exercise program is designed for people with multiple risk factors for developing heart disease. Developed to give participants tools to make lifestyle changes and reduce risk factors, the focus is on making choices and decisions that improve overall health. The safety of nurse-supervised exercise is combined with fun, supportive environment and solidarity of being with others facing the same issues. Once finished, participants walk away with a program to take to a local gym or use at home. 

    • Classes 3 times a week for 6 weeks with personalized exercise prescription and coaching
    • Nurses supervise and monitor exercise classes with ongoing feedback utilizing pulse watches
    • Body fat analysis pre and post program
    • Dietitian consult and food plan recommendations
    • Comprehensive risk factor reduction education and coaching.

    The cost of the program is $198.00 and is not reimbursable by insurance or Partnership Healthplan.


    Non-Monitored Exercise Program or Maintenance Program (formerly known as Phase III)

    Patients who no longer require close monitoring or who have just finished one of the traditional Cardiac or Pulmonary Rehab programs can benefit from the maintenance program. Patients who are familiar with the equipment and routine can continue their program with security of nurse-supervision. With a strong support group type environment, participants help each other stay on track to keep exercising. 

    • Exercise classes 2-3 times a week, usually Tuesdays and Thursdays
    • Nurse-supervised session, with blood pressures monitored before and after class
    • Yearly re-evaluation and goal setting, with report sent to physician 
    • Ongoing education and support from nurses
    • Participants pay a monthly fee for remaining in the program. This program is not covered by insurance or Solano Partnership Healthplan.

    Supervised Exercise Makes the Difference 

      The Cardiac & Pulmonary rehabilitation team develops individualized exercise programs based on physician orders and each participant’s personal exercise experience, fitness level, special needs and limitations.

      A focus on proper exercise technique means programs include stretching, warm-up and cool-down. Specially trained nurses help set individual goals and review progress, guiding participants through personal exercise routines. Nurses electronically monitor heart rate and blood pressure during the sessions, watching closely for any changes. Small group classes allow nurses to focus on safety while patients make progress toward recovery.

    Education Improves Long-term Health 

      Lifestyle choices have a significant impact on recovery and prevention of future heart problems. NorthBay Cardiac and Pulmonary Rehabilitation program staff provide support and encouragement to help patients make healthy changes to last a lifetime. Educational components of the program include:

    Our Programs Are Nationally Certified

    Nationally certified by the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR), NorthBay Cardiac & Pulmonary Rehabilitation meets the gold standard of rehabilitative care for heart patients. AACVPR instituted program certification in 1998 to recognize programs that were meeting the standards in the published Guidelines for Pulmonary and Cardiac Rehabilitation. These documents reflect the highest standard of care and are regularly updated. Programs must demonstrate adherence to these nationally developed guidelines by undergoing the review process to become certified, and must recertify every three years. Those applying for certification are required to complete an extensive checklist related to the daily operation of their program, patient safety, staff competency, and proper policies and procedures.
