Cardiac and Pulmonary Rehabilitation is a
physician-prescribed, nurse-supervised
exercise and education program. Our cardiac and pulmonary team
will guide you in strengthening self-care activities. A nurse will
help you set goals, determine your needs and review your progress.
We also focus on patient and family education, including smoking
cessation, nutrition, cholesterol and stress management, medications and
home exercise programs.

Monday through Friday 6:30a.m. to 3:30
p.m. (No weekend hours.)
Closed only for the following holidays:
New Years, Presidents Day, Memorial Day, 4th of July, Labor
Day, Thanksgiving and Christmas.
Congestive Heart Failure Program
IV Infusion clinic for patients requiring
additional treatment along with a comprehensive educational component to
support the person in controlling this complex disease.

Education Classes Offered by
Cardiac Rehab
The Cardiac Rehabilitation department
offers exercise classes for the following specialized groups:
Cardiac
Patients recovering from Cardiac events
such as heart attacks, post PTCA/stents, Valve surgery or bypass
surgery. Patients frequently admitted with CHF or angina can also
benefit significantly. Individual exercise programs and a comprehensive
education program are given to each participant and are supervised by
registered nurses. Telemetry monitoring during exercise assures safety.
Patients attend 3 days a week for 4 to 12 weeks. Exercise therapy
(Cardiac Rehab) is covered by most insurance if ordered by the
physician. Body fat analysis pre and post program are shared with each
participant.
Pulmonary
Patients with COPD, Asthma or Chronic
Bronchitis are enrolled into a Pulmonary Rehabilitation Program.
Patients attend both educational classes and exercise classes. Each
patient is started at an extremely low level of exercise and work up
from the starting point. RT, Dietary and Rehab nurses work
collaboratively to provide a meaningful program for each patient.
Patients attend 2 education classes, receive a dietary consult, and
exercise 3 days a week for 12 weeks. Exercise therapy/strength and
pulmonary breathing training (Pulmonary Rehab) is covered by most
insurance if ordered by the physician.
Diabetic
Diabetic patients are first enrolled into
the Self Management Diabetic Program through the Health Education
department. Upon completion, they are referred for the second portion of
the program, the Diabetic Exercise program. Patients attend exercise
classes daily, 5 days a week under the supervision of the Cardiac Rehab
staff. Blood sugars are monitored, teaching is reinforced, and dietitian
follow-up is given. Patients begin to practice skills learned in class.
Seeing how their blood sugars respond to diet and exercise give patients
the immediate feedback needed to understand the importance of compliance
to their treatment plan. The Diabetic exercise program is 6 weeks in
length and is a self pay program at $25 per week. We currently have a
grant to provide these classes to Partnership patients free of charge.
Non-Monitored Exercise
Program
Patients who no longer require close
monitoring are offered to join the non-monitored exercise program. This
program is offered 2 times a week at the Cardiac rehab department on
Tuesdays and Thursdays. Patients are re-evaluated and monitored yearly
and a report is sent to their physician for a yearly update. This is a
self pay program and is not insurance reimbursable. This is an extremely
popular program due to the advantage of continued rehab nursing
supervision, support and advice. We currently have approximately 135
members taking advantage of these classes.
What does the future hold?
We are currently looking at other
exercise program options. There has been interest in opening a wellness
program for people who wish to avoid health problems related to
inactivity. We have also been investigating programs specifically for
CHF, peripheral vascular disease and arthritis. Keep your eyes open for
future programs.
