Learn about the services available through Hospice & Bereavement

Our Hospice Team is composed of professionals with expertise caring for the unique physical, social, spiritual and emotional needs of patients and families at the end of life. The team includes the patient's primary care doctor, the hospice medical director, nurses, social workers, home health aides, volunteers, a chaplain, therapists and others as needed. NorthBay Hospice nurses make regular visits to assess the patient and provide pain and symptom management; the frequency depends on the patient’s condition. Nurses also provide education and support for family members and other caregivers. Home health aides make regular visits to provide personal care (such as bathing) and to assist with activities of daily living.
