Make Your First Appointment

Once you have chosen your new primary care doctor, make an appointment by calling NorthBay Center for Primary Care at (707) 646-5500 and press "1" for new patients when prompted.

When you call, be sure to let the representative know if there are any circumstances that might require an appointment sooner, such as if you need refills on prescriptions or are experiencing symptoms that need urgent attention.

As a new patient, you will be asked to complete some forms. You are welcome print, complete and bring the forms with you to your first appointment or plan to arrive 15 minutes early to allow time to fill them out in person.

We understand that plans can change and you may need to reschedule your appointment. Please be sure to review the patient visit guidelines.

Signup for the Patient Portal, MyNorthBayDoc

Don’t forget to ask your patient service representative about our patient portal, MyNorthBayDoc. Sign up and stay connected with your doctors, anytime, anywhere from any device. Schedule appointments online with your primary care doctor, message your care team, request medications, view lab and most test results and access your health records securely. Don’t worry if you forgot to sign-up with our patient service representative, you can also enroll online.

Medical Records

Be sure to transfer your medical records and history so your new doctor has all the information needed to be able to provide you with the most effective and appropriate treatments and services. You can elect to transfer the entire file, select parts, or just a summary. It may take 15 days for your previous provider to process your request, so please submit your request as soon as possible.

Prescriptions

Please bring all prescriptions, in their original bottles, with you to your first appointment. You and your doctor will review your medications, and if needed, a new prescription(s) will be provided. Your prescriptions can be sent to any pharmacy that your insurance provider contracts with. Please be sure to share your preferred pharmacy.

Referral to Specialists

If you are in need of specialty care, your Primary Care doctor will make recommendations and connect you with the NorthBay specialist best suited to treat your specific needs. Our specialists are located throughout Solano County, and many even offer appointments conveniently within our Centers for Primary Care. Because NorthBay Healthcare is a member of the Mayo Clinic Care Network, our patients can even get a second-opinion from national experts without having to leave home. This means the care you receive from NorthBay specialists is enhanced by Mayo Clinic expertise and knowledge.

Urgent Care

When you're not feeling your best, you don’t want to wait to get care. Our team of Emergency-Room-trained physicians and clinicians provide convenient, comfortable, personalized care to get you feeling better faster without long wait times. NorthBay Urgent Care locations are open in Fairfield and Vacaville, with in-person and virtual visits available, seven days a week for treatment on a wide range of conditions from colds and flu to broken bones. Onsite lab, X-Ray and pharmacy is available.

If you are suffering from unexpected bone and joint pain or an injury, same-day access to comprehensive services with our talented orthopedic network is available at our NorthBay Center for Orthopedics in Fairfield.
